The Bilingual Community Health Worker/Navigator (CHW) role is part of the WISEWOMAN Program and is an outreach position to oversee compliance with The Connecticut Early Detection and Prevention Program, the breast and cervical cancer early detection grant (CEDPP), requirements for those patients who have cardiovascular disease risk factors and provide education regarding prevention to decrease risk.
The Community Health Worker is responsible for providing navigation services - works closely with the clinical team to ensure the clients have integrated, comprehensive, and coordinated care, including follow-up with clients from initial identification through closure via phone calls, or community settings where clients can be found.
Qualifications
Experience:
Any combination of 3 years health/social services experience and/or education
Bilingual/bicultural Spanish
Experience in Women’s Health, strongly preferred
Education:
High School diploma or equivalent
Associate Degree, BS or BA in Social Services, strongly preferred
