Southington Care Center features a state-of-the–art rehab pavilion complete with Aqua Therapy and an Occupational Therapy suite that replicates the home setting. Our staff promotes quality of life while fostering maximum well-being, with a wide range of wellness programs.

Generally, the Social Worker will work on short-term patient discharge plans and social service support of short-term residents.   Will also assure residents' psycho-social needs are met and their rights are upheld by assessment, counseling and advocacy as prescribed by professional standards.

Qualifications

• Bachelor of Science, Social Work – Masters preferred
• Experience in discharge planning strongly preferred
• Experience in health care setting with geriatric experience preferred
• Knowledge of regulatory agencies and requirements
