Job Summary:

The Nurse Navigator plans, coordinates and delivers health services to patients receiving care within the department of Neurosurgery, specific to cranial diagnoses.  This includes working collaboratively with other health care providers to assure coordinated care throughout the continuum of care.  The nurse navigator will assist with the coordination of services and will provide support and education throughout the clinical course. She/he will serve as a professional resource for the department of Neurosurgery, specific to cranial diagnoses.

Job Responsibilities:

  • Perform daily patient rounds on all cranial patients; reinforce patient education regarding disease process and rehabilitation goals; serve as liaison between patient and inpatient team
  • Work with the case coordinators on the units to ensure a smooth transition for the patients care; partner with attending’s and inpatient team to ensure timely; Establish direct communication with nurse navigator and case coordinators for cranial patients; serve as liaison between the patient and surgeon, inpatient teams, and outpatient services as determined by patient’s clinical needs
  • Help to create and implement care plan designed by physician including regular office visits, ensure communication with care team, and follow through with treatment plan
  • Nurse navigator will regularly communicate with cranial patients according to set cadence by provider; Coordinates patient’s appointments with specialists and removes barriers to ensure timely access to care; Conducts both in person and virtual/phone nursing visits with patients
  • In collaboration with the surgical scheduling team, manage surgical work up to ensure that all results are communicated to provider and patient is guided through preoperative process smoothly
  • Create curriculum for patient education regarding surgical process and post op care; Implement virtual education group classes for patient; Create and implement support group for families of cranial patients; Facebook group
  • Utilizes current evidence-based practice to develop, implement, revise, and maintain protocols, guidelines, policies, and procedures.
  • Establish and host Case Conference for complex cranial cases

 

 

 

Qualifications

  • BSN
  • Licensed as a professional RN in the state of Connecticut
  • 1-3 years nursing experience
  • Experience with cranial neurosurgery preferred
  • Bilingual – Spanish preferred
  • CPR Certification
