JOB SUMMARY

The Advanced Practice Provider (APP) will provide professional, effective, efficient and appropriate services to HHC NH patients in order to serve the community’s healthcare needs. The APP will follow established standards and practices as a member of the Neighborhood Health team. 

Promote, protect and preserve the health of the public through:

  • Help identify and develop relationships with local community resources
  • Identify community stakeholders in each region
  • Assist with development and dissemination of marketing materials
  • Perform direct local outreach and assist to identify at risk populations
  • promoting healthy lifestyle
  • prevent disease and health problems
  • provide direct care
  • educate community about managing chronic conditions and making healthy choices

Skill in providing excellent customer service and support; strong active listening skills, organizing and prioritizing workload and meeting deadlines; and excellent written and verbal communication.

JOB RESPONSIBILITIES

 

  1. Responsible for assessment, diagnosis, and treatment of chronic illness & disease, acute illness & injuries, infectious & communicable diseases, prescription management, immunization administration as indicated, and referral to appropriate primary and specialty care needs. 
  2. Able to perform minor procedures including suturing of minor lacerations and incision and drainage
  3. Responsible for collaborating with the staff for the day-to-day operations of the clinics:
  • Site set up and break down
  1. Able to provide safe and appropriate basic patient care and administrative duties in order to assist patients in achieving established outcomes by: collecting data points;
  • Vital signs: Weight, Temperature, HR, BP, RR, Pulse Oximetry, Pain,
  • Allergies
  • Medication reconciliation; document pharmacy location if available
  • Past Medical/Surgical/Social/Immunization History
  • Point of Care Testing (POCT)
  1. Ability to delegate responsibilities to appropriate team members based upon an understanding of other’s roles, knowledge and scope of practice
  2. Perform various other duties as assigned to maintain safe & efficient patient flow
  3. Maintains a safe, clean, and orderly environment for patients and colleagues
  4. Contributes to improvement by participating in huddles, training, education and performance improvement activities as directed
  5. Follow the HHC Neighborhood Health clinic policies & procedures

 

  1. Models quality patient/customer services by:
  • Demonstrating a willingness to serve all customers at all times
  • Maintains an awareness of patient needs and utilize the initiative to respond to those needs within skill level
  • Utilizes tact, discretion and compassion in collaborating with other members of the team to ensure quality patient care

 

  1. Initiates appropriate action in urgent situations by adjusting priorities to address immediate customer and/or staff concerns which may include but is not limited to:
  • Use of ambu bag/BVM for respiratory distress
  • Use of AED for cardiac emergency
  • Use of anaphylaxis kit for allergic reactions
  • Activating 911 for EMS transport to higher level of care

Qualifications

Qualifications

Required Education:

  • Graduate of an Accredited Family Nurse Practitioner Program
  • Certified by American Nurses Credentialing Center or American Academy of Nurse Practitioners
  • Shall meet the requirements of the CT Board of Medical Examiners, and be licensed to practice medicine without restrictions in the state of CT.

OR

  • Graduate from Accredited Physician Assistant Program
  • Certification by NCCPA

 Required Experience:

  • Minimum of 3 years as a FNP/PA
  • Minimum of 3 years recent pertinent professional nursing experience in a hospital, medical office, or other clinical setting
  • Minimum of 1 year experience in an environment where children are treated

 

 Preferred Experience:

  • Urgent care experience preferred

Licensure, Certification, Registration Required:

  • Current Connecticut RN and APRN or Connecticut PA-C license
  • Current CT Controlled Substance License
  • Current DEA license
  • Current BLS certification
  • Must meet and maintain HHCs credentialing and privileging requirements
  • Valid CT driver’s license

Licensure, Certification, Registration Preferred:

  • Current ACLS certification
  • B-CON certification (training can be provided)

Language Skills:

  • English, Excellent verbal, written and personal communication skills
  • Bilingual is a plus

Knowledge, Skills and Ability Requirements:

Excellent communication skills demonstrated through written documentation, direct communication and using active listening skills

- Ability to shift with changing priorities and embrace change

- Customer service skills: proactively and promptly responds to customer needs

- Must be willing to travel between HHC regions

- Must be willing to work evening and weekends if necessary
