The Hartford HealthCare Cancer Institute (HHC CI) is a certified member of the Memorial Sloan Kettering (MSK) Cancer Alliance. Through the bi-directional collaboration facilitated by this partnership, community providers are better positioned to provide state-of-the-art, evidence-based cancer care to improve the lives of cancer patients. Together, the Hartford Healthcare Cancer Institute and Memorial Sloan Kettering are staying ahead of cancer.

The Imaging Follow Up Navigator (RN) is responsible for partnering with patients and providers to offer a personalized, individualized approach.  It is essential to the development of good communication between the patient, physicians, Oncology Nurse Navigator, Medical Imaging, and Cancer Center support staff. Responsibilities will include but, will not be limited to:

  • Facilitate scheduling appointments for imaging and follow-ups, streamline paperwork, explain and clarify testing and results and serve as a link in connecting patients with appropriate resources. This position ensures patients and referring physicians are efficiently and proactively notified of results and provides follow up recommendations for incidental findings to help improve patient care.
  • Offer personalized care to patients as well as track interventions and outcomes.
  • Communicate and collaborate with the patient’s referring provider to improve timeliness of care and patient satisfaction.
  • Work in collaboration with the imaging department to assist in preparing case presentations for all cancer conferences related pulmonology.
  • Assess and work in collaboration with Radiology personnel and referring Clinicians to ensure no patient “falls through the cracks” and care is coordinated when follow up is recommended through Imaging.
  • Ensures patients and referring physicians are efficiently and proactively notified of results and provides follow up recommendations for incidental findings to help improve patient care.
  • Work in collaboration with the Oncology Nurse Navigators, thoracic surgeons, and pulmonologists for proper handoff of malignant diagnoses.  

Qualifications

Education:

Graduate of an accredited Registered Nursing program; Bachelor’s preferred

Experience:

Three (3) years recent clinical experience in focus area of Oncology, five (5) years preferred

License and Certification:

•Current Registered Nurse in the State of Connecticut.
