Hartford HealthCare is seeking an experienced, full-time, Nurse Practitioner or Physician Assistant providing primary care to geriatric patients in various settings in Newington and Southington and surrounding area. Care settings include Skilled Nursing, Assisted Living, and home.
Position details:
• Adult Primary Care
• Full-time, weekdays
• Minimal Phone Call Only
• No weekends

Advanced Practitioner (APP) Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s), performs routine physical examinations, and formulates medical, educational, and psychosocial plans to treat patient. In consultation with the physician(s), develops plan for further evaluation of health needs, e.g., referral to other health team members or specialists. Provide appropriate treatment for problems within scope of a Nurse Practitioner (APRN) or Physician assistant (PA) practice as designated by protocols. Identifies problems outside of APRN or PA scope of practice and refers appropriately to physician.
