Hartford HealthCare Rehabilitation Network is the premier rehabilitation provider in Connecticut. We provide rehabilitation services across the continuum - acute care hospitals, long term care facilities, sub-acute rehabilitation, homecare, outpatient centers, and in community settings such as senior centers, work sites, and schools and colleges. We offer over 30 outpatient locations with a variety of specialists, including oncology, women’s health, sports medicine, physical therapy, occupational therapy and speech language, we are known for our commitment to high quality, customer focused outcomes.

 

 

At Jefferson House we provide excellent long-term care and rehabilitation services to seniors in the greater Hartford area for over 120 years. We are comprised of a skilled and caring staff which provides seniors with Nursing Services, Therapeutic Recreation, Spiritual Services, and Social Services, aimed at caring for the whole individual.

 

Under the supervision of a Physical Therapist, Occupational Therapist, Physical Therapy Assistant or Occupational Therapy Assistant performs delegated functions as a member of the rehabilitation team by providing multi-skilled services. This role contributes to the successful operation of the work area, and functions in accordance with all existing regulatory and company standards, policies and procedures. Reports to Site Director/Administrator.

Qualifications

  • Active Cerrtified Nursing Assistant Certification for Connecticut 
  • Minimum of 1 year work experience preferred
  • Knowledge of medical terminology preferred.
  • Computer literacy
  • CPR validation may be required within 3 months of hire depending upon work area specific requirement.
  • Physical effort as required by Position Physical Requirements and Occupational Exposures Form
