-Educates and assists clinical staff and patients in applying knowledge of nutrition, therapeutic diets and nutrition management in the home setting for patients and families.
In general, most of the time will be spent in the following activities:
-Thorough analysis of patients- nutritional status, and makes recommendations and educates clinicians/patients/families on appropriate dietary modifications based upon patients- nutritional assessment.
-Document nutrition consultations, assessments, communication with patients and clinicians in the electronic health record.
-Prioritizes the need for home visits to patients who have complex nutritional needs, including those on enteral and parenteral nutrition, those with non-healing wounds, diabetes and cardiopulmonary diseases.
-Collaborates in development of patient/family plan of care with case manager and/or supervisor.
-Participates in Interdisciplinary Team meetings and case conferences upon request of case manager and/or supervisor
-Maintains knowledge of Agency's services, policies and procedures and community resources
In addition, the position involves other duties such as:
-Participates in the orientation and for professional and paraprofessional staff
-Maintain resources for staff on topics of nutrition and therapeutic diets
-Develops educational materials and programs for clinical and paraprofessional staff, and in collaboration with other clinical specialists on an annual basis and as requested
-Provides individual nutritional counseling/education to VNAHC employees upon request
-Maintain and updates annually resources for staff on topics of nutrition and therapeutic diets
-Works with other Registered Dietitians in area hospitals, clinics and community agencies to provide continuity of care upon request
-Participates on Agency committees and special projects
-Prepares reports on patient outcomes, productivity and other upon request
-Actively participates in the performance improvement process known as H3W.
-Performs other duties as assigned
Licensure: Registration by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics
Education: Bachelor degree in Nutrition or other science, Master's Degree in Public Health or Nutrition
Experience: Two years experience in a hospital, clinic, extended care facility or community health agency
