The Pharmacy Department at Hartford Hospital utilizes a decentralized model with pharmacists embedded on medical teams to advice on optimal drug therapy. We also have an American Society of Health-System Pharmacists (ASHP) accredited Pharmacy Residency program, an active Oncology infusion service, and a Retail/Specialty outpatient pharmacy.

The Ambulatory Care Pharmacy Manger will collaborate as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational, safe, evidence-based and cost-effective therapy.  Responsibilities are and not limited to:

  • Manages the planning, design, development, and implementation of patient-centered and coordinated delivery of clinical ambulatory pharmacy services
  • Develop collaborative drug practice agreements with providers at practices
  • Have direct patient responsibilities including both in-person and centralized management activities
  • Lead medication management performance improvement initiatives
  • Develops metrics and targets related to productivity, clinical outcomes, and satisfaction (patient, provider, employee, and other key stakeholders)
  • Effectively plans and coordinates ambulatory clinical care services with providers and other departments.
  • Participates in councils, meetings, quality improvement teams and other such committees as needed
  • Ensures the efficient utilization of resources to meet productivity and financial goals
  • In collaboration with fellow managers, assures compliance with relevant accreditation and payor standards (eg URAC, ACHC) as they pertain to areas of oversight
  • Provide education, training and work direction to pharmacists and providers as required
  • Develop/maintain competencies for the clinical staff to ensure that the department is meeting its clinical and operational goals
  • Mentor and develop ambulatory pharmacists on clinical projects, guidelines, protocols



Doctor of Pharmacy degree (PharmD) from a US Accredited College of Pharmacy or Bachelor of Science degree in Pharmacy with minimum of three or more years of experience


Three or more years of experience

Preferred: Five or more years of experience as well as prior supervisory/leadership experience.

License and Certification:

Licensed a registered Pharmacist in Connecticut or eligible to receive licensure within 45 days.
