JOB SUMMARY
Provides regional support to stroke program to enhance patient throughout the hospital stay through transition. Identifies patient experience and outcomes.  Utilizes data collected to make informed decisions to drive outcomes.  

Qualifications

Education: Bachelor’s Degree in nursing required. Master’s Degree in nursing or health/business field preferred.

Experience:  At least three years of experience in a clinical area (such as cerebrovascular) or program management. Case management, emergency room nursing or clinical experience in stroke unit or critical care preferred.

Licensure, Certification, Registration: Current Connecticut Registered Nurse license required. ACLS and BLS required.    

Language Skills:•    Highly effective verbal and written communication skills required. 

Knowledge, Skills and Ability Requirements:

•    Ability to work effectively with other departments.
•    Computer literate and database managing experience
•    Ability to manage and organize multiple clerical priorities
•    Ability to work independently
