Job Summary

Under the supervision of a Registered Nurse, or Provider, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice. Position reports to Nurse Manager.

  • Collaborates with members of health care team to assist patients in achieving established outcomes of care
  • Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
  • Documents patient/family responses, and reports variances from anticipated outcomes 
  • Performs therapeutic measures, under the direction of an RN or MD, to assist in achieving established patient care outcomes
  • Contributes to care improvement by participating in meetings, training, in service education, performance improvement activities, as directed
  • Communicates appropriately with identified resources, and reinforces patient/family education Ensures cost-effective utilization of resources within own control, and contributes to cost-effective utilization of overall resources
  • Performs other related duties in support of efficient department operations and superior patient care


Current Connecticut Licensed Practical Nurse license required. Medical/Surgical office experience preferred.

3 years of experience preferred
