Our Social Workers (BSW) utilize their extensive education and training to provide personalized care to our patients in a rewarding environment that promotes autonomy
Assist MSW staff in maintaining compliance to DPH requirements for referral response. Assist patients in connecting to and applying for entitlements/benefits and community resources as needed to meet goals of MSW patient plans of care.
In general, most of the time will be spent in the following activities:
Assist MSW staff with patient and MD phone calls, faxes and HIM documentation in accordance with DPH requirements for referral response.
Works with other Agency staff in determining patient acuity level for timely services and assist with assigning and scheduling the MSW for home visits.
Confers with Agency staff regarding patient emergent needs and consults with Manager and MSW staff to assist in meeting these needs.
Assists patients in obtaining short and long term assistance in the home by educating, referring and completing applications for community resources, entitlements and benefits.
Completes short and long term skilled nursing/rehabilitation placements including completions of PASRR level I and Level of Care documents, locating appropriate facility, assessing payer sources, coordinating with MD offices and maintaining contact/coordination with patients/families and Agency care team staff.
Conducts Telephone Interventions with patients/families as needed.
Maintains thorough knowledge of Agency services, policies, procedures and community resources and maintains online shared resource folder.
Documents all contacts and interventions in EMR.
In Addition, the position involves other duties such as:
Demonstrates effective communication, organizational and time management skills
Follows all agency policies
Attends Agency and departmental education programs and meetings
Actively participates in the performance improvement process known as H3W
Performs other duties as assigned
Bachelor's degree in Social Work from a school accredited by the Council on Social Work Education
One year experience in a health care setting
