Basic Purpose of the Position:
The Primary Care Nurse in the Healthy Minds at Home Program ensures quality and safe delivery of comprehensive short-term acute nursing care to primarily Older Adults with the diagnoses of Dementia, Parkinson Diseases, Anxiety and/or depression. Provides evaluation and assessments and supportive interventions and linkage to the community to maximize their level of functioning.
In general, most of the time will be spent in the following activities:
Evaluation and assessment using Evidenced Based Assessment Tools. Understands and addresses co morbid medical conditions.
Develops, implements, evaluates plan of care. Addresses problems with plan of care, attends and initiates client case conferences, provides coordination of the plan of care and maintains continuity of care with other health professionals assigned to the case and communicates recommendations for additional home health care services to the appropriate individual in a timely manner.
Provides assessment, education and supervision of the individualized, holistic plan of care. Engages with appropriate community resources to ensure a safe continuum of care.
Documents timely and accurately computer entry of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports.
Performs other duties as required.
Demonstrates competent nursing assessment and treatment skills in the provision of services within the Healthy Minds at Home scope of practice.
Demonstrates competent knowledge of medications and current therapies.Demonstrates understanding of nursing process, motivational interviewing. Maintains a positive professional demeanor.
Demonstrates organizational, written, and verbal skills. Demonstrates proficiency in EMR systems and computer skills.
Licensure: Connecticut Registered Nurse
Education: BSN or ASN from a NLN accredited School of Nursing with 2 years recent psychiatric nursing experience in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services OR Master's Degree in Psychiatric/Mental Health Nursing, or related discipline
Experience: One year nursing experience or one year nursing experience in a specialty; medical/surgery or home care, preferred. Computer skills preferred.
