Responsible for gathering necessary information from referral sources, promoting Agency services and functioning as a clinical resource and customer service representative
In general, most of the time will be spent in the following activities:
** Receives referral information and selects appropriate Agency services
** Obtains all payer information relevant to the referred client
** Assists with payer verification process
** Functions as a clinical resource coordinating complex case
** Responds to inquiries regarding Agency services accurately identifying needs of patients, presenting VNAHC services in a positive, problem solving way. Follows-up with phone requests to implement services
** Develops and maintains positive relationships with referral sources, keeping them informed of new and existing services
** Inputs referral data into various billing systems
** Clarifies, verifies and signs-off on physician orders
** Completes referral form to be used by visit staff and Accounts Receivable
** Assists in the orientation of staff and students as assigned
** Directs LPNs in establishing a plan of care and in determining required services
In addition, the position involves other duties such as:
Will actively participate in the performance improvement process known as H3W.
Performs other duties as assigned
Licensure: Registered Nurse with a license to practice in the State of Connecticut
Education: Graduate of an Associate's or diploma program in nursing; graduate of an NLN-approved BSN program, preferred
Experience: Home care experience preferred. One year Intake/Admissions experience preferred. Knowledge of insurance verification, benefit determination, managed care contracts and government reimbursement criteria preferred.
