Opportunity for an experienced Physician Assistant or Nurse Practitioner to provide Per Diem outpatient services for weight loss patients in Glastonbury, Meriden, Farmington, and Bloomfield.

 

Some highlights of this opportunity include:

  • The per diem advanced practitioner will work in an ambulatory setting providing care patients wanting to lose weight to improve their quality of health. Responsibilities include seeing patients for follow ups and developing a treatment plan that includes prescribing, treating, and educating patients. Additional responsibilities will include patient care coordination with collaborating physicians and Patient Navigators, care plan documentation, work groups, participating in various hospital committees, and tracking of patient work up’s.

Qualifications

Eligibility to be licensed as a Physician Assistant (PA), Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP) is required. Experience practicing as an Advanced Practitioner is required.
