The creation of the HHC System Support Office recognizes the work of a large and growing group of employees whose responsibilities are continually evolving so that we and our departments now work on behalf of the system as a whole, rather than a single member organization.

The Performance Improvement coordinator is a professional role supporting the Quality and Safety Management Department of the Heart and Vascular Institute in conducting ongoing performance improvement activities for the designated site of care and/or region(s). The primary functions of this position consist of leading performance/process improvement for targeted initiatives, oversight of collection/abstraction and aggregation of data and the analysis of data /metrics to assess performance. The position collaborates with key stakeholders/ departments in developing best practice standards to ensure reliable safe design of care processes and workflows. The incumbent is a subject matter expert in regulatory (CMS, JC, DPH), payer performance programs (QHIP), improvement science and high reliability concepts. Core to this role is functioning as a consultant to ensure compliance with all required agencies. This candidate works closely with clinical teams and Medical staff to develop practices, peer review, quality, and safety efforts that are aligned with HHC’s core values. The candidate will have proficiency with facilitating project teams as well as organizational department efforts to improve systems and process within the organization. The individual should have experience in independent project management, efficiency, and an understanding of the principles surrounding performance improvement, quality of care, outcomes measurement and analysis and health care cost measurement and containment. They need to be able to communicate effective verbally and in writing. Able to work flexible hours. Experience strong computer skills: with microcomputer software for database management and graphics (MS Office, Word, Excel, Access, PowerPoint, SPC charts) required. Comfort with Internet databases. Experience with data and outcomes analysis important
CT RN license, Bachelor’s Degree
Or Masters in related field if non RN (MHA, MPH)
Minimum 5 years clinical experience
Experience with database/spreadsheets
