Basic Purpose of the Position:
** As part of a person centered care model and under the direction of the RN, this role will provide skilled behavioral health and dementia nursing care to clients in home and community based settings ensuring the delivery of outcome based, high quality, cost effective care. This role will collaborate with all team members to achieve exceptional outcomes.
In general, most of the time will be spent in the following activities:
** Provide intermittent skilled behavioral health nursing services in accordance with the established plan of care under the direction of a RN.
** Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.
** Records daily progress in EMR
** Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases
** Communicates/conferences with primary nurse and other disciplines
** Consults with physician regarding client needs and plan of care
** Assists in evaluation of care given by paraprofessional staff
** Participates on agency committees and special projects
** Attends and participates in in-service education program
** Participates in Team meetings and case conferences as well as IDT and LEAN daily management process
** Seeks consultation and guidance from supervisor and responds to requests timely
** Maintains productivity standard
In addition, the position involves other duties such as:
Will actively participate in the performance improvement process known as H3W
Performs other duties as assigned
Current and Valid LPN license
Valid CT drivers license & good driving history
High school diploma or GED required; and successful completion of a CT state approved Licensed Practical Nurse certificate program
Computer skills including but not limited to Outlook, Microsoft Word and Excel. Excellent organizational and time management skills. Ability to provide excellent customer service.
