For more than 118 years, Backus Hospital has been the primary source of healthcare services for much of Eastern Connecticut. We are home to the only trauma center in New London and Windham counties, and are the only area hospital with LIFE STAR helicopter services. We deliver and coordinate a continuum of high-quality healthcare with the goal of improving the health of our communities.
In partnership with the multidisciplinary team of providers and the Heart & Vascular Institute, the Limb Preservation Program Nurse Navigator is responsible for the coordination of care in the outpatient/ambulatory setting for patients that are receiving care as part of the program. The nurse navigator will collaborate with other disciplines and community providers to support the patient as they move through the care system with the goal of reducing the rate of limb loss in the region and improving patient experience.
Education: Bachelor’s Degree in nursing required. Master’s Degree in health related or business field preferred.
Experience: At least three (3) years of experience in a clinical area (such as wound care or vascular surgery) or program management required. Three years of in wound care and/or vascular surgery with program development/management experience preferred.
Licensure, Certification, Registration: Current Connecticut Registered Nurse license required.
Language Skills: Highly effective verbal and written communication skills.
Knowledge, Skills and Ability Requirements
• Clinical experience caring for patients with vascular disease
• Ability to work effectively with patients, providers
• Computer literate and database managing experience
