Shift Details:

Basic Purpose of the Position:

 

-Consults clinical staff in the overall delivery of services and maintenance of clinical care standards for clients in area of specialization; Develops, directs, and manages program operations; develops, maintains and implements program goals and objectives.

 

In general, most of the time will be spent in the following activities:

 

-Makes home visits to provide clinical support and assists nursing staff in patient assessment, identification of diagnosis, and development of a comprehensive nursing care plan 

 

-Manages a client caseload as needed

 

-Provides consultation to staff identifying and planning interventions designed to solve complex problems involving client care

 

-Consults with clinical education area to provide programs aimed to improve clinical competencies of direct service staff

 

-Researches new information related to specialty and communicates to staff

 

-Develops an interdisciplinary approach to client care with Agency staff

 

-Provides guidance to nursing staff and nursing students working on individual projects and research, including own clinical research activities

 

-Follows trends in specialty area and home care industry; makes recommendations for incorporation into program

 

 

-Hires, trains, coaches and evaluates staff.  Determines staffing needs and coordinates hiring to meet service demands.

 

-Accountable for team performance in achieving desired clinical and operational performance measures

 

-Identifies and facilitates professional development needs and competency of staff

 

In addition, the position involves other duties such as:

 

Assists in acquiring grants and special funding for expansion of clinical services; seeks financial resources for clinical research

 

Serves as a community liaison presenting informational programs to heighten awareness of needs of specialty population

 

           Assists in orientation of new employees

 

           Will actively participate in the performance improvement process known as H3W.

 

           Performs other duties as assigned

Qualifications

QUALIFICATIONS

 

Licensure: Registered Nurse with a current State of Connecticut License

 

Education: A Diploma in nursing. Bachelors Degree in nursing preferred; MSN in the specialty area preferred; specialty certification, preferred

 

Experience: Three years of nursing experience in clinical specialty area; two years experience in home care preferred
