Our staff of highly trained professionals treats patients holistically, considering family and living situations for the most appropriate care plan.

Job Description: Providing personal hygiene and grooming care to clients; Assistance with toileting, ambulation, etc.; Providing meals for clients' nutritional needs; Reporting changes in clients' condition to supervisor, or client care coordinator; Performing routine housekeeping and home management duties; Recording each client's care on care plan and submitting care plan in a timely manner; Assisting with providing emotional support for the clients and their families; Assisting with routine therapy exercises; Participating in case conferences as requested; Attends all required in-service education sessions.

Qualifications

• Minimum 1 year work experience, preferably in home care, hospital or other healthcare facility
• Driver's License and Auto Insurance Required
• CNA certification preferred
