The Health Equity division, part of the Health Equity, Diversity and Inclusion Department, is responsible for health equity promotion and health disparity reduction in the communities that Hartford HealthCare serves. Additionally, the team provides subject matter expertise in equitable strategies, community health & engagement, advocacy, health equity analytics, bias reduction, and diversity, equity and inclusion practices. The Health Equity division partners closely with both clinical and administrative areas to assess, track, and implement improvement methods that create an equitable environment of care for all.

The Health Equity Specialist is someone who is passionate about health equity and is instrumental in working on system-wide health equity projects that improve outcomes, reduce disparities, and reduce unconscious bias. In addition to working within the department, the Specialist serves as a health equity subject matter expert on various health equity initiatives throughout the system. Responsibilities include but, are not limited to:

  • Support and conducts regular Health Equity Workgroup meetings with key constituents to build relationships and share best practices and health equity tactics that improve health outcomes.
  • Monitors, coordinates and communicates the strategic objectives of health equity KPIs across the system to optimize performance/results.
  • Consults leaders and stakeholders on improvement opportunities in providing equitable, safe and high-quality care to patients.
  • Works to identify opportunities for continuous improvement, standardization, and health disparity reduction.
  • Identifies communications strategies for dissemination of equitable outcomes research and recommendations.
  • Partners with diverse teams and leaders across the organization on system-wide health equity programs and initiatives.
  • Assists in the development of community outreach and engagement content and methodologies
  • Aggregates and assists with the analysis of internal and external health equity data.
  • Compiles reports about incidents, events, or updates pertinent to equitable care.



MHA, MPH, MBA, MSN or advanced degree with major coursework in healthcare management preferred, or either related field or equivalent work experience.


• 2+ years relevant experience in a healthcare environment.
•Demonstrated previous experience in health equity, health improvement, health disparity reduction or related field.

•Understands and respects issues related to health equity, conscious language, diversity, and inclusion
•Deep understanding of the needs and challenges of historically underserved and marginalized communities
•Knowledge of project and change management principles and terms.
