Basic Purpose of the Position:
Current and valid RN license
Coordinates communication between contracted monitoring company and clinical staff.
In general, most of the time will be spent in the following activities:
Distributing clinical and non-clinical information from monitoring company to appropriate clinical teams, managers and staff
Documents in electronic medical record, calls, case conference, clinical notes related to patient’s care and TM service
Collaborates installations, troubleshoots and discharge of patients with the Tele-monitor assistants :
Conferences with Case manager/ MD office regarding TM services
Coordination of email/voicemail alerts from contracted monitoring company.
Reviews Life Stream daily:
Collection of weekly deployment data, re-hospitalizations of TM patients reports, other metrics as assigned to Quality
Participates in any investigations/reviews of adverse outcomes for TM patients
Actively participates in the improvement process known as H3W to identify TM service line needs and development.
Performs other duties as assigned
Qualifications
Licensure: Current and Valid RN license
Valid CT drivers license & good driving history
Education: High school diploma or GED required; and successful completion of a CT state approved Licensed RN preferred
Experience: Computer skills including but not limited to Outlook, Microsoft Word and Excel. Excellent organizational and time management skills. Ability to provide excellent customer service.
