Shift Details:

Hartford HealthCare is seeking a full-time, Urgent Care, APRN or PA in the Fairfield county area.



Rotation between the following sites of service:


·       1055 Post Road, Fairfield, CT


·       401 Monroe Turnpike, Monroe, CT


·       15 Armstrong Road, Shelton, CT


·       3272 Main Street Stratford, CT


·       900 White Plains Road, Trumbull, CT


The ideal candidate will possess:


·       A passion for providing patient-centered, urgent care focused on quality, prevention and education to patients


·       At least one year of post-graduate practice experience in a similar setting


·       Training and experience treating patients of all ages; birth through geriatrics


·       A willingness to work closely and collaboratively within an interdisciplinary team


·       An unrestricted State of Connecticut APRN or PA license (or eligible)


Position details:


  • Full-time, rotating schedule

  • No on-call

  • Urgent Care Center hours:  Mondays – Fridays, 8:00AM – 8:00PM, Saturdays – Sundays 9:00AM – 5:00PM

Advanced Practitioner Role Scope:


   
Under established guidelines and in collaboration with the practice physician(s), performs routine physical examinations, and formulates medical, educational, and psychosocial plans to treat patient. In consultation with the physician(s), develops plan for further evaluation of health needs, e.g., referral to other health team members or specialists. Provides appropriate treatment for problems within scope of a Nurse Practitioner (APRN) or Physician Assistant (PA) practice as designated by protocols.  Identifies problems outside of Nurse Practitioner (APRN) or Physician Assistant (PA) scope of practice and refers appropriately to physician.


 


With the knowledge and consent, and the direction of the physician, a Nurse Practitioner (APRN) or Physician Assistant (PA)  may carry out the following:


  1. Evaluate the status of any assigned patient by history and physical exam.  Obtain complete medical history.
  2. Managing to specific medical needs for patients in Options to Health Clinic. These patients have complex psychiatric and emotional issues.
  3. Perform complete physical examination.
  4. Order appropriate lab tests, imaging studies, radiographic and other cardiac imaging studies.
  5. Review patient charts, write progress notes, and complete all other required documentation in the Electronic Medical Record.
  6. Complete record keeping, observe strict confidentiality, and attend continuing medical conferences and patient review sessions.
  7. Keep work area safe and orderly.
  8. Follow Universal Precautions.
  9. Performs other related duties as required.


Requirements:


  • Graduate of an accredited Nurse Practitioner or Physician Assistant program preferred

  • One year of post-graduate practice experience in a similar setting

  • Unrestricted State of Connecticut medical license (or eligible) as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA)
