St. Vincent's cardiac program is recognized regionally and nationally for innovation and outstanding clinical results. The Heart Failure Program has achieved Gold Plus and Target HF awards from the American Heart Association for the Get With the Guidelines Program. We are the primary teaching hospital for the Frank Netter School of Medicine at Quinnipiac University.

 

Advanced Practitioner Role Scope  

The Heart Failure Advanced Practitioners provide direct care and disease management for acute and chronic heart failure patients in an outpatient ambulatory clinic setting. In addition to seeing and treating patients in the clinic, responsibilities include remote monitoring of heart failure patients, including those with implanted cardiac devices, collaboration with cardiologists and our structural heart team, and referral to other specialists for comorbidities.

Qualifications

Advanced Practitioner Requirements

  • Graduate of an accredited Nurse Practitioner Master’s program preferred
  • Licensed, or eligible to be licensed, as an Advanced Practice Registered Nurse (APRN) in the state of Connecticut
  • National Board Certification, or eligible to be certified, as an outpatient Nurse Practitioner
  • Cardiology or Congestive Heart Failure experience required
