Job Summary

Post Liver Transplant Coordinator needed at Hartford Hospital outpatient clinic. Typically 8:00AM-4:30PM with some flexibility as needed.

Responsibilities may include:
*Coordinating the care of liver transplant recipients
*Identifies and responds to recipient/family education, psychosocial and economic needs of post kidney transplant recipients.
*Reinforces education and follow-up requirements to facilitate positive outcome.
*Assess recipient compliance to medication and medical regimen.
*Maintaining knowledge and working within the guidelines of UNOS and CMS requirements.
*Manages scheduling and flow of clinic. Works with Medical Assistants in clinic and coordinates physician visit with patients.
*Completes data capture, analysis and reporting to UNOS registry by making data available to data coordinators.
*Actively participates is QUAPI.
*Participates in community activities related to transplant including seminars, presentations, and support group activities.
*Demonstrate proficiency and continued growth in the field of transplantation by working to obtain and maintain CCTC certification.




Qualifications

 

• Current Connecticut registered Nurse license.
• 2-4 years recent clinical acute care experience.
• Experience in transplantation, medical or surgical preferred.
• CCTC required within 3 years of hire.
