This Endocarditis Nurse Navigator serves as a member of the multidisciplinary care team, responsible for coordinating the individualized care of endocarditis patients throughout the continuum of care in order to increase and assure timely access to healthcare services. They should demonstrate use of clinical skills and expertise in the area of diagnosis, treatment and psychosocial impact of disease, demonstrates excellent customer service skills including superior telephonic and written communication skills. They will functions as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services. And will participates in community education (educational outreach programs, workshops, seminars, support services).
Bachelor of Science in Nursing
5 years clinical nursing experience;
3 years clinical experience with substance use disorder (SUD) patients
Licensure, Certification, Registration:
Connecticut Registered Nurse license
Certification in specialty preferred
Spanish fluency highly preferred
Knowledge, Skills and Ability Requirements:
Knowledge of Cardiology and substance use disorders (SUD)/principles of addiction recovery highly preferred
