The Health Navigator position is grant-funded by the Connecticut Early Detection & Prevention Program at Hartford HealthCare. The Connecticut Early Detection & Prevention Program (CEDPP) is a comprehensive screening program available throughout Connecticut for medically underserved women. The primary objective of the program is to significantly increase the number of women who receive breast and cervical cancer screening, diagnostic and treatment referral services. In addition to breast and cervical screening, the CEDPP has funding to cover cardiovascular screening and diagnostic colonoscopy.
Key areas of responsibility include, but are not limited to:
Qualifications
Education and Experience:
· Bachelor’s Degree strongly preferred with three or more years of experience working in community health. High School diploma with 10 years’ experience in community health in lieu of a degree.
Knowledge, Skills and Ability:
· Self-directed and motivated individual with flexibility and collegiality.
· Strong organizational, analytical, problem-solving and communication skills in a highly visible and responsible position.
· Experience with multi-tasking.
· Proficient in speaking and writing in English and Spanish.
· Experience using medical databases, Microsoft Office Suite and Outlook.
