Position Details

Hartford Healthcare is seeking a Nurse Practitioner or Physician Assistant for highly regarded, collegial, and collaborative Transplant Clinic practice. The APP would see patients in outpatient setting.

Role: Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistant/Physician Associates (PA-C)

Location: Outpatient Offices are located in Hartford

Setting: Outpatient responsibilities in clinic

Schedule: 40 hours per week; Occasional weekends and holidays are required

 

Nurse Practitioner and Physician Assistant Role Scope:

·        The Outpatient Post-Transplant Midlevel Practitioner will provide comprehensive medical care for patients who have undergone kidney or other solid organ transplants.

·        They will work in an outpatient setting and focus on managing post-transplant care, including monitoring patient progress, managing immunosuppressive therapy, and coordinating follow-up care. 

·        Conduct outpatient clinic visits for post-transplant patients, including taking medical histories, performing physical examinations, and ordering diagnostic tests. 

·        Address acute and chronic post-transplant issues, including infection prevention, rejection monitoring, and medication side effects. 

·        Collaborate with transplant surgeons, nephrologists, nurses, social workers, and other healthcare professionals to ensure comprehensive patient care. 

·        Coordinate follow-up care, including scheduling appointments, managing referrals, and communicating with other specialists. 

·        Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and communication with other healthcare providers. 

·        Ensure all documentation meets regulatory requirements and institutional policies. Participate in quality improvement initiatives and contribute to developing best practices for post-transplant care. Stay current with advancements in transplant medicine through continuing education and professional development opportunities. 

Qualifications

·        Licensure: Unrestricted State of Connecticut Medical License (or eligible)

·        Training: APRN (Family, Pediatric, Adult/Geri Nurse Practitioner) or PA

·        Experience:  At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) a plus – not a must.
