Job Summary

Under the Direction of the Inpatient RN Director & Social Work Supervisor the Inpatient Social Worker Case Manager will work collaboratively with the IP Psychiatry Patient Care Team and take the lead role in organizing & providing case management services on a 17-Bed Adult unit.  The IP Patient Care Team is comprised of MD, ARPN, MSN, RN, Social Workers, Recreational Therapist and Patient Care Technicians.  The Social Work Case Manager has primary responsibility for arranging follow up care and supports for all patients discharged from the unit.  

Job Responsibilities

• Conducts a comprehensive case management needs assessment that results in the development and implementation of a care/discharge plan including coordination with outside agencies consults/referrals to remove barriers to follow up care.
• The Social Work Care Manager will also provide backup for Clinical Social Workers on the unit by providing other direct patient care including psychosocial assessments, individual, group, and family treatment
  • Provides education to patient/family with respect to meeting established goals
• Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
• Maintains timely patient-centered documentation that reflect the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies. 
• Participates in administrative and development activities including team meetings, clinical supervision
• Supports the development of hospital and/or unit-based programming to meet the clinical and strategic needs of the patients and the milieu.

Qualifications

• Master's degree in Social Work
• Active LMSW required
• Two years clinical experience in a behavioral health setting
