Job Schedule: Per DiemNurse Practitioner or Physician Assistant – Palliative Care
Position Details:
Hartford HealthCare is seeking a per diem Nurse Practitioner (APRN) or Physician Assistant (PA) for Outpatient Palliative Care in the town of Plainville, CT.
Role: Open to Nurse Practitioners (FNP) and Physician Assistants (PA)
Location: Plainville, CT
Setting: Outpatient Palliative Care Per Diem
Schedule: Average of 4-8 Hours of Clinical Time per week
Call schedule: No Call required

Nurse Practitioners and Physician Assistants Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s), the Nurse Practitioner or Physician Assistant will provide consultative services to patients living with serious illness, with the goal to reduce physical discomfort and make living with serious and chronic illness easier for both patient and caregiver.
Qualifications
Requirements:
· Licensure: Unrestricted State of Connecticut Medical License (or eligible)
· Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
· Experience: At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA)
