Job Description
Under the supervision of the Medical Director, the Medical Weight Loss Program Coordinator is responsible for assisting with planning and leading various meetings related to the bariatrics program. This includes meeting with Providers to discuss program initiates and program updates. Coordination includes setting up meetings with Providers to discuss program initiatives, program updates.

Candidate must be comfortable with learning about nutrition. Administrative duties include but are not limited to participation in committees, projects as assigned, clinical policy development, standard work development, scheduling of education and on-boarding of staff.  Leadership is demonstrated by role modeling best practice in clinical care and environment of care arenas. Also leads work in development of new programs regarding diet education.

  • Assess and Evaluates clinical operations through rounding to ensure standardization of nursing practice across sites for improved patient outcomes. Provide the clinical support staff with timely, honest and respectful feedback, both positive as well as areas of opportunity. Validates staff completed assignments. 
  • Educate clinical team and patients on dietary recommendations created by department dietician. Orders dietary information from digital print shop. Conducts meetings to provide dietary education.
  • Triages patient calls and refill prescriptions. Perform other related duties as needed.

Qualifications

  • Completion of an accredited LPN program required
  • CT LPN licensure required
  • At least 2 years of providing patient care in an ambulatory care setting.
  • Strong administrative and computer skills.
  •  Experience with Excel, Word and PP preferred.
