East Region Advanced Practitioner Operations and Program Manager
The Advanced Practice Provider will work throughout the East Region, including Backus and Windham Hospitals and the Medical Group offices. Advanced Practice Provider Manager will provide quality, holistic and cost-effective advanced practice care for patients, families, and others in the general cardiology, electrophysiology and heart failure populations and participate in the surgical and medical management of patients to achieve and/or maintain optimal health status, manage disease and reduce risk of illness. Duties will include, patient management, crisis intervention and discharge planning on the medical, critical care and emergency department units.
In addition, the East Region Advanced Practice Provider Operations and Program Manager will be responsible for overseeing the practice and personnel of all Advanced Practice Providers in the East Region for general cardiology. Other responsibilities will include recruitment and retention of advanced practice providers, yearly and ongoing performance evaluations, quality improvement initiatives, new program development and standardization of advanced professional practice across the East Region. The Norwich APP Lead will report to the East Region Advanced Practitioner Operations and Program Manager. The Advanced Practitioner Operations and Program Manager will report to the Norwich Practice Manager with a dotted line to the East Region Director of HVI.

Qualifications
Advanced Practitioner Qualifications
· Graduate of an accredited Physician Assistant / Nurse Practitioner Master’s program preferred
· Licensed, or eligible to be licensed, as a Physician Assistant / Advanced Practice Registered Nurse (APRN) in the state of Connecticut
· Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse through AACN or ANCC.
· National Board Certification as an Acute Care Advanced Practice Registered Nurse
