Hartford HealthCare is actively looking for a Care Coordinator (LMSW) to join their dynamic team with Case Coordination in New Britain, CT.

The Hospital of Central Connecticut is an acute-care community teaching hospital, we provide comprehensive inpatient and outpatient services in general medicine and surgery, as well as a wide variety of specialties. Care Management is a collaborative practice model including patients, nurses, social workers, physicians, healthcare team members, caregivers and the community.  The Care Management process encompasses communication and facilitates care along a continuum through effective resource coordination.  The goals of the Care Management Team include the achievement of optimal health, access to care and appropriate utilization of resources balanced with the patient’s right to self-determination.

 

 

Qualifications

Qualifications - External

  • Masters in Social Work from an accredited school of Social Work. 
  • Minimum of 3 years of Medical Social Work experience. Acute Care experience preferred.
  • Maintains current license as Masters in Social Work in the State of CT (LMSW). Case Management certification ACM or CCM Strongly preferred.
  •  Ability to effectively utilize a variety of Microsoft Office, Outlook and Epic software, Morrissey and Web based programs required.
  • Candidates with experience in groups and individual work with youth 12-18 are preferred.
