Hartford HealthCare Rehabilitation Network is the premier rehabilitation provider in Connecticut. We provide rehabilitation services across the continuum - acute care hospitals, long term care facilities, sub-acute rehabilitation, homecare, outpatient centers, and in community settings such as senior centers, work sites, and schools and colleges. We offer over 40 outpatient locations offering physical, occupational, and speech therapy, in specialties including orthopedics, neuroscience, oncology, women’s health, sports medicine, vestibular, and more. We are known for our commitment to high quality, customer focused outcomes.
Under the clinical supervision of an Occupational Therapist, the Certified Occupational Therapist Assistant administers Occupational Therapy treatments to patients. Provides assistance to the Occupational Therapist by supplying objective measures to be utilized by the Occupational Therapist in evaluation of patient’s level of function and determination of treatment strategies.
· Assists the Occupational Therapist with aspects of patient assessment in the areas of activities of daily living, leisure/play, and productivity/work. Identifies contributing factors in the areas of psychosocial skills, cognition and sensorimotor components. Recognizes physiological, psychological, and age-related changes in the patient.
· Assists with the planning of the Occupational Therapy plan of care, implements treatment by utilizing appropriate interventions and age specific treatment through selected activities and interventions. Collaborates with the Occupational Therapist to revise treatment when needed, instructing patient/family through verbal written and audiovisual materials and participating in discharge planning to meet patient needs throughout the continuum of care.
· Confers with Occupational Therapist, and consults with other members of the Health Care Team members, and assesses progress toward goals in order to meet the needs of patients.
· Documents all patient and professional interactions such as evaluations, assessments, progress toward goals, and discharge summaries to maintain continuity of patient care for department standards, JCAHO and outside regulatory and insurance agencies.
