Hartford HealthCare is actively looking for a Medical Case Aide to join their dynamic team with the Department of Medicine in New Britain, CT.

Job Summary - Under the supervision of the Chief of Infectious Diseases the Medical Case Aide will provide case management to clients with HIV infection who have multiple medical and psychosocial needs.

  • Refer clients to Social Service agencies for programs such as Food Stamps, Eviction Prevention and Energy Assistance etc.
  • Provide education on the disability programs provided by the Social Security Administration such as SSI and SSDI
  • Refer patients to community resources for transportation, housing/shelters, food services, and employment
  • Assist clients in arranging transportation services
  • Assist clients  in scheduling medical/dental and other necessary appointment
  • Complete all paperwork and data entry in accordance with current policies
  • Act as an advocate for the clients with community based organizations and public services to reduce barriers and enhance care
  • Assist clients in completing forms for various benefits

Qualifications

Requirements

  • Associate’s Degree Required, BA in Social Work or Human Services preferred

  • Experience with case management in a healthcare or public welfare setting required

  • Must be Bi-lingual (Spanish) 

Knowledge, Skills and Ability Requirements:

·         Knowledge of HIV, substance abuse and psychiatric illnesses required

·         Proficiency in Word, Excel and knowledge of CareWare software a plus

·         Ability to deal efficiently and effectively with a wide variety of individuals
