Job Summary
Under the administrative direction of the Manager, Case Management, demonstrated ability to manage, instruct and collaborate with patient care and social service staff. Familiarity with health care and social work needs to inpatients, outpatients and community to address the needs of age specific categories of patients according to hospital, governmental, and/or regulatory policies, rules or regulations, and according to the NASW Code of Ethics and practices of Social Work.
Qualifications
EXPERIENCE: Five (5) years experience in the field of Social Work. Three (3)years experience in the field of Health Care. Management experience preferred.

LICENSURE/CERTIFICATION: Membership in good standing with NASW and/or Academy of Certified Social Workers. License for social work practice in state of origin. Eligible for Connecticut Clinical Social Work (LCSW) license preferred.
QUALIFICATIONS: Bachelor’s Degree in Social Work (BSW) from an accredited School of Social Work in clinical with knowledge of health care and social work theory and practice. Must possess counseling skills. Basic knowledge of computers. Understanding of the range of psychosocial and discharge planning services as well as knowledge of community resources needed by patients, outpatients and community members of all ages and stages of growth development. Ability to obtain and interpret information relevant to patient needs. Bilingual (Spanish/English) abilities preferred.
