Physician Assistant / Nurse Practitioner Role Scope   
Hartford HealthCare's – Central Region (THOCC and MidState Medical Center) is seeking an Advanced Practitioner (APP) who will work in collaboration with the attending physician and members of the interdisciplinary care team to deliver expert palliative care services to patients and their families living with chronic serious or acute life threatening illness, or those nearing end of life.  The APP conducts a comprehensive assessment of the patient and family to promote informed decision-making, clarification of goals of care, and advance care planning. The APP is an expert in pain and symptom management, seeking to optimize patient function and quality of life within the limits of the disease process, while considering factors such as fear, communication barriers, economic issues and cultural traditions.

  • Provides comprehensive palliative care patient assessment for the diagnosis, treatment, and management of acute or chronic health illness or symptoms
  • Collaborates with members of the interdisciplinary health care team to serve as patient advocate to achieve goals and preferences for care identified by the patient/family
  • Ensures excellent communications skills to lead patient/family goals of care conversations, assist patients/families with health care decision making, and conduct advance care planning discussions
  • Participate in the development of clinical practice guidelines and policies/procedures in support of optimizing quality palliative care
  • Prepare and maintain accurate patient records to support expert professional practice and reimbursement for services provided
  • Provides palliative care education and training for providers, staff, and students throughout the care continuum
  • Is knowledgeable of hospice regulations and standards of care and offers hospice services to patients and family as appropriate
  • Participates in palliative care QI/PI activities
  • Collaborate with HHC system colleagues to improve knowledge, skills and access to palliative care services across the continuum through education and community outreach activities
  • Contributes to the development of meaningful goals and objectives for Palliative Care services that align with organizational vision and mission

Shift: Full-time, Monday through Friday

Qualifications

Advanced Practitioner Requirements

  • Graduate of an accredited Physician Assistant Program required
  • Licensed, or eligible to be licensed, as a Physician Assistant in the state of Connecticut
  • Certification or eligible to be certified as a Physician Assistant
  • Advanced Practice Registered Nurse in the state of Connecticut
  • Preferred Nurse Practitioner specialty: Acute Care, Adult Care, Geriatrics or Family
  • Preferred National Board Certification in Palliative Care
