Under the Direction of the Inpatient Nurse Manager & Inpatient Associate Medical Director, the Inpatient Psychiatric Social Worker will work collaboratively with the IP Psychiatry Patient Care Team to provide comprehensive clinical social worker services to patients hospitalized for acute psychiatric needs. The IP Patient Care Team is comprised of MD, ARPN, MSN, RN, Recreational Therapist and Patient Care Technicians. The Social Work will assess and support patients admitted to the unit, provide clinical recommendations to the team, provide individual, family, and group therapy, and assist in discharge planning.
Conduct comprehensive patient and family assessments for new admissions to the unit that meet DPH/CMS guidelines
Provides individual, family, and group therapy as requested by the providers or IP Nurse Manager
Prepares appropriate medical documentation to meet regulatory requirements of licensing and third party agencies as well as internal policies
Ensures patient and family understand the provider’s diagnosis and planned course of treatment. Assesses and communicates clinical and psychosocial treatment needs to the treatment team. Identifies gaps / barriers to care and works together with case management to transition patient from Inpatient to follow up care
Contributes clinical recommendations to provider team regarding therapeutic interventions with patients related to the patient’s diagnosis/treatment history to assist caring for the patient in the milieu with the goal of mitigating safety risks for the patient and staff
Actively participates in quality and safety initiatives to support the unit as assigned by the IP Nurse Manager. Assignments may include CHH or Behavioral Health Network initiatives. Assigned work may include committees, aftercare calls to patients
Support Case Management, Recreational/Activity Therapy activities when assigned staff is not available
Conducts Emergency Department based Crisis Assessments and Ambulatory Assessments when assigned by the Administrative Director, Medical Director, or Associate Medical Director
Provides training and oversite to per diem social workers and master’s level social work interns
Facilitates and/or actively supports units efforts to maintain wellbeing of staff through debriefing/clinical support, and professional education
Perform other related duties as required
Master's Degree in Social Work
2+ years of experience
LMSW is required
Demonstrated competency in providing psychosocial assessments, input on clinical diagnosis, and ability to provide individual, family, and group therapy as an independent practitioner
