Job Summary:

Job Summary: Seeking an exceptional nurse leader to manage a dynamic team of inpatient nurse case coordinators, as well as an administrative team who help support daily operations and patient throughput. The ideal candidate would be experienced in Case Management, and be a well-respected clinician in this field. This leader should have exceptional organizational and communication skills, as well as the proven ability to successfully collaborate across multi-functional teams to drive optimal clinical and financial outcomes related to patient flow and transition planning.

Reports to the Nurse Director of Case Coordination.

Job Responsibilities:

  • Ensures compliance with regulatory requirements related to the CMS Conditions of Participation for Discharge Planning.
  • Conducts recruitment efforts, hiring, performance management, oversees training for new staff as well as ongoing competencies, and the development and evaluation of staff on an ongoing basis.
  • Identifies and resolves issues affecting the timely transition of patients.
  • Systematically evaluates the quality and effectiveness of case management practice.
  • Provides leadership and oversight to staff and has accountability ensuring all quality, financial, and service benchmarks as well as compliance with case management requirements related to efficient patient throughput.
  • Responsible for the day to day operations related to inpatient transition planning, and through unit rounding with the teams, provides just-in-time coaching to front line staff to support all activities related to patient flow, and assists with throughput barrier resolution.
  • Other Duties as assigned





  • Current/active CT RN License.
  • Bachelor’s degree in Nursing required; Master’s degree in Nursing or related field preferred
  • Certification in Case Management preferred (Certification within 1 year of hire would be required)
  • Minimum 5 years Acute Hospital Case Management experience including Utilization Review and Transition Planning.
  • Proficient time management & prioritization skills.
  • Excellent communication skills, written and verbal.
  • Motivated, self-starter, enthusiastic personality.
  • Adept problem-solving skills.
