Job Summary:

Provides nursing care, medical case management to patient with multiple medical and psychosocial needs.  Applies nursing process to meet patient’s needs. Coordinate psychosocial support services with community based social and substance abuse agencies and case managers.

Increases patient entry into and retention within medical care while improving each patient’s overall quality of life through the development and monitoring of practical and realistic plans of care

 

Key areas of responsibility

  • Triage and assignment of incoming patients based on acuity scale of need; maintain a waitlist as appropriate
  • Perform a comprehensive nursing assessment to identify patient needs with particular emphasis on medical-related needs and other needs that may jeopardize a patient’s optimal health outcomes.  Evaluate and develop the plan of care while partnering with the multidisciplinary team, patient and family as appropriate;
  • Routinely evaluate each patient’s medical status including CD4 counts, viral loads, medication changes and type and severity of symptoms.
  • Provide health maintenance nursing duties that include:  administration of vaccines, injections, medication adherence via pillbox program, patient/family education, and facilitate transitions of care
  • Maintain complete, orderly and updated patient files in compliance with HIPAA regulations, Ryan White Part A procedures and State of Connecticut Ryan White MCM Standards of Care. Completes program reports as necessary.
  • Advocate on behalf of patients for the removal of barriers to quality of medical care
  • Serve on agency committees as requested by manager
  • Participates in special projects and perform other related duties as assigned

 

 

Qualifications

  • ASN required /BSN preferred
  • Valid CT State Registered Nurse license.
  • 5 years of case management experience in a healthcare or public welfare setting.
  • Bilingual (English/Spanish) preferred

 

  • Working knowledge of government entitlement programs, AIDS assistance programs and community social service programs
  • Excellent communication [written, verbal and listening] and interpersonal skills.
  • Strong organizational and time management skills are essential
  • Excellent problem-solving and critical thinking skills
