Job Summary:
The Patient Safety Manager is responsible for the oversight of Hartford HealthCare’s safety program and provides a strong influence towards our journey to high reliability. The Patient Safety Manager possesses a solid clinical background and is responsible for the advancement of the culture of safety and “just culture” on a day to day basis. Responsibilities include but, are not limited to:

  • Provide safety program oversight, safety assessments, analysis of data, implementation of systems and processes along with coordination of action plans to improve safety.
  • Coordinates the high reliability process, serious safety event team and root cause analyses related to clinical performance improvement and patient safety.
  • Conduct ongoing monitoring of safety practices through assessments, surveys and studies to identify opportunities for improvement in safe practices and reduction of risks and system-related issues that create barriers to safe practice in areas that have impact on patient care. 
  • Simplify and standardize systems and processes throughout the organization. Using root cause analysis, failure modes effects analysis, continuous quality improvement and Lean tools to evaluate near miss and actual events and resolved safety concerns.
  • Provide patient safety consultation, guidance and expertise to problem solve for system and individual issues for all levels of the organization.
  • Develop constant awareness of the current state of systems and processes that effect patient care through reporting to leaders and staff. 
  • Maintain strong working relationships and communication with involved departments to encourage reporting of events and near miss opportunities for improvement.
  • Provide a strong clinical presence and be readily accessible to clinical and non-clinical staff.
  • Work closely with the Medical Staff Office and Medical Staff Leaders to ensure consistent implementation and adherence to safe practice.
  • Design, present, and evaluate the effectiveness of educational safety programs used to train all levels of the organization to help identify and respond to system failures. 
  • Develop, maintain and update patient safety related East Region and HHC regional policies and procedures.
  • Chair the Patient Safety Committee.
  • Collaborate with Quality Department and Risk Department and others as appropriate to ensure seamless approach to patient safety and risk reduction.
  • Educate staff on the safety implications of regulatory issues.
  • Actively participate in survey preparation and promote compliance with State, Federal, TJC, and other accrediting agency regulations, standards and survey process.



  • Registered Nurse preferred, other clinical healthcare professionals considered. Bachelors of Science Degree is required.
  • Master's Degree in Nursing, Health Administration, Management or related discipline strongly preferred.


  • Five or more years of management experience preferred. 
  • Participation in relevant committees related to Safety or Quality.
  • Background in Patient Safety with Safety Certification

Licensure, Certification, Registration:

  • RN with CT License or relevant healthcare profession license as applicable.
  • CPHQ and/or CPPS Certification is preferred.
