Position Summary:

The Community Health Outreach Case Manager is part of an interdisciplinary team that includes, but is not limited to, medical, case management and community development.  Serves as patient advocate and assists in identification and improvement of service delivery. Ensures patients remain or become engaged in services, and are educated about self-care issues.  Develops community relationships with the support of the community development team assuring access to care barriers are identified and addressed.

Key Responsibilities:

  • Provide in person community based outreach/engagement to the local emergency room, other formal/informal providers working with or responding to high risk patients.  Foster close working relationships with individuals in the provider, faith, business, government, and community in order to become recognized, accepted and utilized as a resource for individuals.
  • Provide integrated case management support for individuals with language and literacy barriers. Case management support will include collateral contacts/referrals related to: housing; insurance/benefits; medical care; transportation; facilitating communication between providers; setting up and facilitating attendance at appointments; preliminary health education/resources; childcare problems; routine non-clinical correspondence; and miscellaneous contact with family members and community providers.
  • Assist in access to care to encourage and foster relationship between CHH and community.
  • Consult with the medical staff, nursing staff and ancillary department staff to eliminate barriers to the efficient delivery of care. Identify service delivery problems and potential for effective patient management intervention.
  • Assist Care Manager with coordination of patient care in the community, when the patient is hospitalized, and during transition from hospital to home.
  • Meet customers in the emergency department to engage with individuals and make available community outreach services to facilitate the successful access to and engagement with services recommended during emergency department visits. 
  • Advocate for patients when barriers to care exist including language and literacy barriers, access to transportation, problems with insurance coverage, childcare problems, appointment scheduling conflict, etc.
  • Identify cultural, language or other issues inpatient encounters, offer translation, advocacy and facilitation in conjunction with clinical and case management staff
  • Attend appointments with the customer whenever indicated to provide support and linkages to other providers also involved in the customer’s heath.
  • Assist customers in completing benefits applications for state assistance, financial aid and other miscellaneous entitlement programs.  Assist customers with completing application paperwork for employment, courts, state agencies and local non-profit programs.

Qualifications

Requirements:

  • BA/BS in sociology, psychology, social work, or related field required
  • Community based outreach experience, previous experience providing health education required. Previous healthcare and health insurance experience preferred.
  • Bilingual in Spanish required
