Provides overall delivery of TM services and maintenance of clinical tele-monitoring equipment for patient use            

Responsibilities include the following activities:

 Home visits to delivery, set-up and instruct patient and family in using the TM equipment 

 Provides consultation to staff identifying and planning interventions designed to solve TM problems involving patients care

 Assists in providing educational programs for TM program aimed to improve clinical knowledge of direct service staff

 Participates in an interdisciplinary approach to client care for patient using of TM equipment

 Participates in educational programs to train and coach new agency staff. 

 Maintains and logs equipment per guidelines for quality control and maintenance of equipment

 Coordinates home visits and discharges of patients from the TM program

 Will actively participate in the performance improvement process known as H3W.

 Performs other duties as assigned

Qualifications

Education:

High School Diploma, Associates degree preferred

 

Experience:

One- two years’ experience in home care preferred
