Job Summary

 

This Nurse Coordinator will work collaboratively as a member of the Bone and Joint team, with multidisciplinary approach, implements and manages strategic initiates of Service Line which supports sustainable improved processes with quality patient outcomes and service growth within an engaging environment for a cohort of patients.

 

Key areas of responsibility

 

  • Throughout the surgical continuum of care, for future or current programs, creates processes and makes process changes based on measurable patient outcomes. Organizes and Facilitates Service Line meetings, in collaboration with BJI leadership.  Collaborates with the BJI Clinical and Service Line Leaders and participants to assess key outcomes from defined outcome measures. 
  • Provides leadership and support to the nurse navigator role with standards of care and performances regarding transition determination, complex patient disposition, and appropriate services. Oversees patient engagement platforms, Force Therapeutics, with surgeons’ offices, nursing teams, and other inpatient and outpatient healthcare teams.
  • Utilizes current evidence-based best practice to develop, implement, revise, and maintain protocols, guidelines, policies, procedures and assessment of tools specific to service line.
  • In collaboration with members of the Healthcare Team, coordinates patients through the continuum of care by providing clinical expertise resulting in patient and physician satisfaction
  • Serves as an interdepartmental liaison among and between numerous care provider for a specific service line patient population across the continuum of care, including, but not limited to, surgeons office staff, advanced practitioners, orthopedic resident, physicians, nurses, physical therapists and other healthcare members.
  • Creates and organizes community education outreach programs, workshops, seminars and support services. 
  • Provides management and leadership oversights of Joint Commission accreditations and site visits, serving as point person/facilitator of visits.  Tracks disease specific metrics.
  • In collaboration with other healthcare team members creates and facilitates the Patient Engagement Committees

 

Qualifications

Education

Bachelor of Science in Nursing

Masters (Preferred)

 

Experience

Three (3) years clinical nursing experience

Service Line and/or operational experience (Preferred)

 

Licensure, Certification, Registration

Active CT Registered Nurse license

 

Knowledge, Skills and Ability Requirements

  • Knowledge of clinical operational management.
  • Strong communication [written and verbal] and customer service skills with ability to communicate effectively with diverse audiences including professional staff, patients, community, and other service agencies.
  • Demonstrates leadership and problem solving skills
  • Self-directed, organized, with an acute awareness to details
  • Ability to collect, analyze, and interpret clinical data, PROMS, Joint Commission and other regulatory data
  • Flexibility with strong ability to adapt to change
  • Demonstrates strong interpersonal skills with a high level of initiative and follow through capability.
