POSITION DESCRIPTION
Basic Purpose of the Position:
As part of the immersion program, the immersion visiting nurse will provide skilled nursing care to clients in home and community based settings ensuring the delivery of outcome based, high quality, cost effective care. This role will collaborate with all team members to achieve exceptional outcomes.
In general, most of the time will be spent in the following activities:
Makes follow-up visits to provide skilled care to assess disease progress and evaluates response to treatment; provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability; coordinates care with providers
Maintains a client record utilizing the Agency's standard for documentation
Carries out the plan of care under the guidance of the Clinical Development Specialist
Teaches patients and reinforces client/caregiver teaching and education
Refers, collaborates and coordinates all client care services. Consults with physician regarding client needs and plan of care
Communicates / conferences with Nurse Care Coordinator and all involved in the patient’s care
Delegates and directs components of the established plan of care to LPNs and HCAs under the guidance of the Clinical Development Specialist
Refers client/family to other community resources as needed
Attends and participates in in-service education program
Demonstrates active engagement and participates with the immersion visiting nurse cohort and immersion nurse required learning
Seeks consultation and guidance from supervisor
Participates in the evaluation of care given by paraprofessional staff
Participates in health care programs
Completes reporting requirements and responds to requests in a timely manner
Participates in client care conferences, staff and group meetings, IDT and LEAN daily management process
Maintains productivity standards according to immersion program core curriculum
Adopts and demonstrates H3W Leadership Behaviors
Maintains knowledge of and compliance with Federal, State and accreditation regulations as well as Agency policies and
procedures

Qualifications
Licensure: Registered Nurse with a license to practice in the State of Connecticut
Education: Graduate of an Associate's or diploma program in nursing; graduate of NLN approved BSN program, preferred
Experience: Computer skills required. Healthcare experience preferred.
