JOB SUMMARY |

As part of the person centered care model, this role will develops/coordinates agency wide programs for prevention and management of skin breakdown. ii. Provides expert consultation and/or hands-on care for individual patients with acute or chronic wounds. iii. Establishes/coordinates multi-disciplinary, comprehensive management plans. v. Coordinates care for populations of patients, identifying trends, and developing standardized prevention and treatment protocols and refining protocols based on outcomes.. Develops and implements educational programs for patients, staff and other health care providers.. Coordinates quality improvement initiatives to optimize skin and wound outcomes. vii. Advocates for the patient with skin and wound care needs and for the professional |

JOB RESPONSIBILITIES |

Key Areas of Responsibility |

The Wound Program Specialist will act as consultant to help coordinate all client care services required by the patient to achieve outcomes. Informs the physician/wound clinics of changes in client needs, wounds that require change to the plan of care for wound care treatments and disease management
|
