Job Description:
Provides holistic, quality and cost-effective nursing care for patients/families and/or others in the pre-hospitalization population within a unit, service or other primary area of practice; focuses on discharge planning which includes identification of risk factors and education. Applies the nursing process within the context of the organization's Framework for Professional Practice. Functions as clinically competent/expert. Delegates and supervises work appropriately. Reports to Director, Care Continuum.
In this position the nurse is calling the pre procedure and pre surgical patients and completing their profiles in EPIC. It is a mostly telephonic position, with minimal direct patient contact. Some patient contact occurs in the Pre admission testing area when patients come in for pre op EKG or blood work. The phone call is the first contact the patient has with nursing in the pre-operative /pre procedure phase, so making a good impression and having excellent interpersonal skills is required.
Appropriately assesses and plans patient care by: accurately and reliably obtaining, assessing and interpreting data; identifying and documenting problems and expected outcomes based on a plan of care for complex, multi-varianced population of patients; utilizing scientific knowledge base and appropriate physiological/psychological/social/cultural/spiritual concepts; organizing care and interventions; and identifying appropriate resources in order to meet individual and family health goals.
Provides phlebotomy, EKG and demonstrates excellent communication skills and understands the role of the RN in the patient experience. (Will train for phlebotomy)
Qualifications
• CT RN License
• BSN Required
• 3 years’ acute care experience required
