The Manager of Care Coordination will supervise Case Managers, Social Workers, Care Coordination Associates, and Clerical Support Staff.

Under the general supervision of the Director of Care Coordination, The Manager of Care Coordination is responsible for the daily operations of inpatient care coordination. Provides leadership, direction and guidance to staff in the provision of care coordination services. Assists the Director of Care Coordination in achieving departmental and organizational goals and objectives.

Accountable for daily operations of care coordination activities and ensures compliance with state and federal regulatory requirements in addition to Joint Commission standards.

Supervises care coordination staff and contributes to development of performance standards and departmental initiates that support organizational goals.

Interfaces with internal departments, medical staff, community partners, and third party reviewers to ensure regulatory and contractual requirements as well as achieve optimal operational efficiency.

Develops and maintains departmental policies, procedures, and annual educational activities.

Performs annual performance evaluations and provides ongoing coaching and mentoring of staff.

Interviews and recruitment of new care coordination personnel.

Participates in the development of clinical guidelines and their incorporation into the care coordination process.

Maintains departmental standards, policies, procedures and protocols.
Analyzes care coordination outcome data to identify and make recommendations related to specific populations or diagnoses.

Qualifications

Active CT RN License.
• Bachelor’s degree in Nursing or healthcare related field.
• Minimum 5 years Acute Case Management experience including Utilization Review and Transition of Care Planning.
• Ability to motivate and facilitate activities of disparate and multi-disciplinary groups.
Preferred Qualifications:
• Certified Case Manager (CCM) preferred.
• Minimum 2 years management experience in Acute Case Management.
• Proficient time management & prioritization skills.
• Excellent communication skills, written and verbal.
• Motivated, self-starter, enthusiastic personality.
• Adept problem solving skills.
