The RN Nurse Navigator will be responsible for the disease management of chronic disease patients in an acute care hospital setting and providing case management services to this population post discharge. Navigator will also follow up with phone calls to the patients in the community and assure understanding of their post-acute care needs. Excellent communication skills, ability to work within an interdisciplinary team and organization skills are strongly recommended.
Current CT RN License with 3 - 5 years clinical experience required, BSN strongly preferred. Candidate with previous experience with utilization management and managed care. Excellent written and verbal communication skills and patient assessment skills required.
