Position Summary:

The Resource Coordinator is responsible for assessing the socioeconomic and psychosocial needs of clients and connecting them to community based resources. A vital component of this role is working with the Hartford HealthCare System at large, as well as, community agencies to assist with resource allocation and identification of eligible services for patients. The Resource Coordinator is able to build strong relationships within the Hartford HealthCare System, as well as the community, as potential referral sources. This position is ideal for someone with entry level healthcare education and healthcare work background that is passionate about changing healthcare and helping patients.

Position Responsibilities:

Key Areas of Responsibility

  • Processes referrals to ICP Community Care Management team
  • Outreaches to patients telephonically to assess community resource and other needs
  • Documents in patient’s electronic health record and communicates outcome of patient outreach with community care managers, providers, and other care team members
  • Identifies resources within HHC system and larger community to address patient care needs
  • Establishes and maintains favorable relationships with hospitals, social service agencies, physicians and other health care providers, senior centers, community and professional groups, and other possible referral sources
  • Performs other related duties as assigned to support the ambulatory care management team and other system partners, including, but not limited to, developing, producing and distributing both internal-facing and external-facing reports, participating in projects and pilots, collaborating with team members and system partners to strengthen transition of care systems using best practices.

Qualifications

Requirements and Specifications:

Education

  • Bachelor’s degree in the Health and Human Services, Gerontology, public health or related field- OR- a minimum of three years of experience in a related health field

Experience

  • 3 years experience in case management, discharge planning, community health work, medical assisting or resource coordination

Licensure, Certification, Registration

  • Mental Health First Aid Certified (certification training will be provided within 6 months of hire if needed)

Language Skills

  • Minimum: Excellent communication skills both written and verbal
  • Preferred: Bi-lingual

Knowledge, Skills and Ability Requirements:

  • Proficient in Microsoft Office applications (e.g., Word, Excel, Outlook, PowerPoint).
  • Familiarity with electronic medical record software is preferred.
  • Knowledge of the local community and the services available within it.
  • Approachable with excellent listening skills, tact and the available ability to offer support and encouragement.
  • Ability to work independently and productively in a high volume, multifaceted work environment
  • Detail oriented with an ability to manage multiple task assignments in a highly organized fashion.
  • Exceptional customer service skills.
  • Possesses a commitment to holistic care in meeting the needs of clients.
  • Be a reliable and effective team player.
  • Adaptability to changing organizational or team priorities.
  • Knowledge and appreciation of cultural diversity and low literacy issues in care provision.
  • Adheres to HIPAA requirements.
  • Ability to obtain own transportation between multiple sites.
